Join us for a two-part workshop series on Advance Care Planning (ACP), presented by Hospice. This free event is designed to help you understand the importance of ACP and guide you through creating your own personalized plan.
Part 1: Learn what ACP is, why it matters, and how to start planning for your future healthcare needs. Offered at 2 times and 2 locations on February 3rd.
Part 2: February 10th 9 - 10:30 a.m. Get one-on-one assistance with completing your ACP plan. Appointments for Part 2 (February 10th) will be made at the Seminar.
